There are two options for applying to become a Citizen Review Board Member:
1. Fill out and submit our new paperless online application by clicking here.
2. Download the application packet in Portable Document Format (PDF), fill it out, print and sign it, and send it to our office by fax (503-986-5859) or by mail (Attn: Volunteer Resource Coordinator, Citizen Review Board, 1163 State Street, Salem, OR 97301). Download the application here. The PDF version requires the free Adobe Acrobat Reader software.
Once your application is received, you will be contacted within one week by the Volunteer Resource Coordinator, who will process your application and send you information on the next steps in the application process.
